When child-welfare agencies fail in their assessment, the consequences can be considerable and are often irreversible. A new book, called “Out of Harm’s Way: Creating an Effective Child Welfare System,” will be released in the spring. It collects the results of decades studying the problems of this sector and outlines solutions, such as the use of risk analysis as it is done by insurance companies.
