Participants of the telemedicine program for type 2 diabetes of Frauenhofer FIT are supported in monitoring their health condition. In real time, information collected by glucose meter, pedometer and other devices, are recorded and processed in the portal. This data can then be turned into comprehensible graphs. The overall goal is to improve the quality of life of people with diabetes 2 and to cut on health care spending. The portal is designed in such way that it can easily be adopted for other medical conditions.
